Every healthcare professional must be open and honest with patients when something goes wrong with their treatment or care which causes, or has the potential to cause, harm or distress.
In such circumstances, the Duty of Candour procedure requires that the patient is informed, an apology is offered, appropriate support or remedial action is provided, and a full explanation of what happened and any potential effects is given.

There were no incidents that met the threshold for Duty of Candour within Quantum Health during this period.
As no Duty of Candour incidents were identified during this reporting year, there were no specific actions or changes required as a result of Duty of Candour procedures.
However, Quantum Health continues to promote a culture of openness, transparency, and learning. Staff are regularly reminded of their professional responsibilities under the Duty of Candour and are encouraged to report and reflect on any concerns, near misses, or adverse events through appropriate internal processes.
This report is produced in line with the Duty of Candour (Scotland) Regulations and is reviewed and updated annually.
